New to the Portal?

If this is your first time using the portal:

1. Request an invitation from our office (an invitation is sent to the email address we have on file).
2. Open the invitation email and click the activation link.
3. Verify your identity using your date of birth and phone number (or Social Security Number if prompted).
4. Create your password and log in.

Through the Patient Portal You Can:

• View upcoming appointments
• Complete registration and intake paperwork before your visit
• Securely message your healthcare provider
• View lab results and visit summaries
• Request prescription refills (when appropriate)
• Pay your bill online
• Update your health information
