What services do you provide?
We offer advanced wound care, mobile primary care, and transitional care management (TCM) to support patients at home.


Do you coordinate with home health?

Yes, frequently. We help to provide medical oversight while home health handles routine care. We bill separate from home health services.


Do you come to the patient, or is this clinic-based?
We provide all of our care services in the home, assisted living, or facility, and offer in clinic wound visits when appropriate.


How do I know if advanced wound care is the right level of care for me (or my patient)?
If wounds aren’t healing, progress has stalled, or follow-up after discharge is unclear, this is likely the right level of care. We step in when standard approaches aren’t enough.


What makes your model different from traditional clinics?
We bring provider-level care directly to the patient, allowing for more time, better assessment of real-life conditions, and faster adjustments to care.


Do you handle supplies, orders, and coordination?
Yes. We manage wound care supplies, treatment orders, and communication with home health and other providers to reduce delays and confusion. For TCM & PCP we assist with medication and DME ordering as well.


Can you see patients who are hard to place or difficult to manage?
Yes. We specialize in complex and underserved patients, including those with access barriers or multiple comorbidities.
