At Alpine Basin Care, we provide specialized, provider-led wound care focused on healing complex and chronic wounds that require more than routine dressing changes.
Advanced wound care goes beyond what is typically provided by home health agencies. Our team includes Nurse Practitioners & Registered Nurses with advanced wound certifications who collaborate to diagnose, treat, and actively manage wounds using medical and procedural interventions.
What we do:
- Full wound evaluations with medical diagnosis and treatment planning
- Sharp debridement (removal of non-viable tissue to promote healing)
- Infection identification and management
- Coordination with vascular, infectious disease, and surgical specialists
- Advanced dressings and biologic skin substitutes if appropriate
How we’re different from home health:
Home health plays an important role in ongoing care, but they follow provider treatment orders and focus on routine treatment. Alpine Basin Care acts as the
treating provider, meaning we:
- Make medical decisions and adjust treatment plans in real time
- Perform procedures like sharp debridement in the home or clinic
- Direct and coordinate care across all settings
- Assist in writing orders for home health to follow
We often work alongside home health, not instead of it, providing the medical oversight needed to accelerate healing and work toward the desired patient outcomes.
Referral requirement:
For some insurances referrals may be required, they can be sent from Primary Care, Hospitals, Skilled Nursing, Home Health Agencies and more.

Transitional Care Management (TCM)
Transitional Care Management is a short-term medical service designed to support patients after discharge from a hospital, skilled nursing facility (SNF), or long-term acute care hospital (LTACH).
This is one of the most critical times in a patient’s recovery. Without proper follow-up, many patients experience complications that lead to avoidable readmissions.
What TCM includes:
- Timely follow-up after discharge (within days, not weeks)
- Medication reconciliation and safety review
- Coordination with specialists, home health, and caregivers
- Monitoring for early signs of complications
- Ongoing provider oversight during the first 30 days post-discharge
Referral requirement:
TCM services require a
referral from a discharging facility,
such as:
- Hospital
- Skilled Nursing Facility (SNF)
- Long-Term Acute Care Hospital (LTACH)

Mobile Primary Care
Alpine Basin Care offers mobile primary care services, bringing comprehensive healthcare directly to patients in their homes.
This model is ideal for patients who have difficulty accessing traditional clinics due to mobility challenges, transportation barriers, or complex medical needs.
What mobile primary care includes:
- Routine check-ups and chronic disease management
- Diagnosis and treatment of acute illnesses
- Preventive care and health screenings
- Medication management and adjustments
- Coordination with specialists and other healthcare services
